My pocket Coodinator - is a student project for Sunnaas Rehabilitation Hospital and Larvik municipality. It’s a tool to raise the voice of the next of kin who is easelly forgotten in the rehabilitation process of the patient .
"My Pocket coordinator" is a digital / physical folder where the next of kin gathers all information in one place and has access to tools that make the transition easier. Through conversation tools, the Pocket Coordinator creates a relationship between the next of kin, specialist hospital and municipality.
Today, the next of kins are not taken care of to a great extent and they are given a lot of responsibility. Through the project, we want to relieve the next of kin both mentally and physically. This is important because the next of kin`s health affects the patient and vice versa.
Through the project, we want to prevent burnout off the next of kin, thus it is important to take measures early. The service already starts at the specialist hospital and follows up with the next of kin in the municipality.
Consists of: a digital folder with various tools for relief, coordination, information, and care.

Concept
"My pocket coordinator" Which is a is physical booklet (we think it has big potential to be done digitally as well) where the next of kin has access to tools that make the process from Sunnas to Larvik easier, and it’s a place where next of kin can gather important information and take notes.
The activities are facilitated by healthcare workers at specialised hospitals (sunnas) and the municipality of Larvik during designated meetings

We envision that the tools in the booklet could take place in different stages of the journey to support the next of kin.
The service already starts at the specialized hospital and follows up with the next of kin back at home in hands of the municipality.
